Responsibilities:

• Perform focused and ongoing patient assessments and report changes to the RN or physician

• Administer medications and treatments as prescribed (PO, IM, SQ, IV where permitted by state scope)

• Monitor and document vital signs, intake/output, and patient responses to care

• Assist with development and implementation of patient care plans

• Provide wound care, catheter care, tracheostomy care, and other skilled nursing procedures

• Collect specimens and assist with diagnostic testing

• Accurately transcribe, verify, and carry out physician orders

• Maintain complete and timely documentation in EMR

• Educate patients and families on health conditions, medications, and care plans

• Collaborate with RNs, physicians, and interdisciplinary healthcare team

• Ensure patient safety, infection control, and compliance with facility policies
